Confluencia de dos pandemias: COVID-19 e infección por VIH. Implicaciones en la clínica, tratamiento y prevención

RESUMEN

La actual pandemia por el nuevo coronavirus SARS-Cov-2 está ocasionando una crisis sanitaria global por su elevada contagiosidad y morbi-mortalidad. Nuestro objetivo es revisar la literatura científica sobre el origen y características virológicas del SARS-CoV-2, la patogenia, manifestaciones clínicas, diagnóstico y tratamiento de la enfermedad COVID-19, con especial énfasis en el impacto de esta enfermedad en personas que viven con VIH (PVVIH). Estudios clínicos e in vitro sugieren que algunos fármacos antirretrovirales (FAR) podrían tener actividad en la prevención, evolución y tratamiento de la COVID-19. Las PVVIH con buen control inmuno-virológico, no parecen tener peor pronóstico que la población general, aunque algunos autores sugieren que las PVVIH con CD4<200/µL o con viremia no suprimida podrían tener un mayor riesgo de desarrollar COVID-19 grave. El envejecimiento prematuro de la población VIH y la mayor frecuencia de comorbilidades podrían empeorar el pronóstico de la COVID-19 en PVVIH.

Las guías clínicas sobre tratamiento antirretroviral (TAR) durante la actual pandemia recomiendan:

  1. Utilizar FAR con escasas interacciones medicamentosas, de alta barrera genética, bien tolerados y con escasa toxicidad;
  2. Espaciar las visitas y las pruebas de laboratorio;
  3. Utilizar la telemedicina,
  4. No suspender o modificar el TAR si no es necesario;
  5. Revisar las interacciones medicamentosas relevantes entre los FAR y fármacos utilizados frente a la COVID-19.

Son necesarios estudios clínicos, con metodologías adecuadas, que evalúen la evolución de la COVID-19 en PVVIH y el posible valor terapéutico de determinados FAR en la prevención y tratamiento de la COVID-19.

ABSTRACT

The current pandemic due to the new coronavirus SARS-Cov-2 is generating a global health crisis by its high infective capacity and mortality. Our purpose is to review the scientific literature about the origin and virology of SARS-CoV-2, and the pathogenesis, clinical manifestations, diagnosis and treatment of COVID-19 disease, with special emphasis on the impact of this disease in people living with HIV (PLHIV). Clinical and in vitro studies suggest that some antiretroviral drugs (ARD) may have activity in the prevention, outcome and treatment of COVID-19. PLHIV with good immuno-virological control of HIV do not appear to have a worse prognosis than the general population. However, some authors suggest that PLHIV with CD4 <200/µL or with detectable viremia should have an increased risk of developing severe COVID-19. The premature aging of the HIV population and the higher frequency of comorbidities could worsen the prognosis of COVID.19 in PLHIV.

Clinical guidelines on antiretroviral treatment (ART) during the current pandemic recommend:

  1. use ARD with few drug interactions, high genetic barrier, well-tolerated and low toxicity;
  2. space visits and laboratory tests;
  3. use telemedicine;
  4. do not discontinue or modify ART if not necessary;
  5. and review the relevant drug interactions between the ARD and those used against COVID-19.

Clinical studies, with adequate methodologies, are necessary to evaluate the impact of COVID-19 on PLHIV and the possible therapeutic value of certain ARD in the prevention and treatment of COVID-19.
